These forms are available to NCTM Affiliates to submit requests, information, and reports to NCTM.
Affiliation Application
Request an Affiliate Packet which includes an application, a checklist, constitution guidelines, and the Affiliate Relations Guide.
Officer Details Form
Submit Affiliate officers' information. The Affiliate NCTM representative is responsible for submitting updated officers' information to NCTM after an election or when officers are appointed.
Annual Report
The president of each NCTM Affiliate is responsible for submitting an annual report to NCTM either the month before his/her term ends or on the anniversary of his/her term, if term is more than one year.
Affiliate Book Commission (ABC)
New program for Affiliates to provide NCTM publications at an event at a discount for attendees and the Affiliates will receive commission on sales.
Affiliate Renewal Form
Pay your renewal dues by April 20 to receive your discount. All payments must be received by June 1 or your affiliate status will be considered “Not In Good Standing” and services will be suspended.
